Kindergarten registration for the 2020-2021 school year is coming up.  Registration for all Live Oak elementary schools will be from June 1 – June 4 at Suwannee Primary School.  Registration will be appointment only.  Please call one of the following numbers to schedule an appointment: SPS – (386) 647-4316 (Mrs. Kim Steichen) or (386) 647-4600 (ask for Mrs. Flowers or Mrs. Herron).

To register, children must turn 5 years old on or before September 1, 2020.  Prior to your appointment, please complete the school choice survey (https://www.surveymonkey.com/r/2020-2021ElementaryChoice).  Students enrolled in a Suwannee County School District (SCSD) VPK program during the 2019-20 school year need to register during this time but you do not need to complete a school choice survey.

Be sure to bring the following items to the appointment:

• Child’s birth certificate
• Copy of a physical dated within one year of 2020-21 starting date (Aug. 10) or doctor appointment card
• Current immunization record
• Child’s social security card
• Custodial parent must enroll child (bring driver’s license)
• Proof of residency (mortgage statement, lease agreement, water/electric bill)

During the appointment, the parent will complete a SCSD enrollment packet, have documents reviewed by the registrar, and the incoming student will be given a basic screener (letters, shapes, colors etc.).  Plan on at least a 30 minute appointment.  Due to COVID-19 please limit the people attending the appointment to the kindergarten student and custodial parent/guardian.
