Parents have the primary responsibility to assure the health and well-being of their child(ren), and schools can also play a partnering role. Due to the “Parents’ Bill of Rights,” section 1014.06, Florida Statutes - Parental Consent for Health Care Services, parents must give written consent for school nurses to provide health care services to minor students. This consent form DOES NOT AUTHORIZE invasive screening or procedures (e.g., COVID-19 testing, blood draw, vaccines, etc.), preventative health care, medication administration, mental health counseling, physical therapy, occupational therapy, or other services that have traditionally required specific parental consent.
If a consent form is not on record/file with the Suwannee County School District, or if you choose “No” to School Health Services, your child will not receive any of the above services, except in the event of an emergency. If your child gets sick or injured (anything that requires first aid or nursing services), you will be called to come pick him/her up from school. Consent forms will be posted to the FOCUS Parent Portal soon. If you do not currently have Parent Portal access, please register or reset your password immediately at the link below so that you may opt in or out of School Health Services for your child.
